This notice explains how the Hendricks Dental Studio website handles information. It applies to this public website and its callback-request form.

Information the website may process
Callback-request form
When you request a callback, the form asks for your name, phone number, email address, preferred callback time and a general appointment category. The website does not ask for symptoms, diagnoses, insurance details or other health information.
Routine server records
Like most websites, the hosting server may create technical logs containing information such as an IP address, browser type, requested page, date and time, and error information. These records are generally used for security, troubleshooting and operation of the website.
Cookie-free spam and rate protection
The form uses a hidden field, a minimum completion-time check and a short server-side rate limit. For the rate limit, the server temporarily uses a one-way salted value derived from the submitting IP address. The website does not place a cookie or browser-storage identifier for this purpose.
How submitted information is used
Form information is used to contact you about your appointment request, respond to your inquiry, protect the form from misuse and maintain the website. It is not used for advertising or behavioral profiling.
How the callback request is delivered
The form sends the request through the practice’s configured email server. The website does not save the submission in a public-facing website database. Once the message reaches the practice’s email system, it may be retained according to the practice’s operational, legal and records-management requirements.
Please do not submit health information
The callback form is for scheduling only. Do not enter medical, dental, insurance, payment or other sensitive information. Call the office at (904) 396-1023 to discuss private details.
This website notice is separate from the practice’s HIPAA Notice of Privacy Practices. Contact the office for a copy of that notice or for questions about how patient information is handled.
External destinations
The map displayed on the contact section is a locally stored static image. Google is not contacted when the page loads. Choosing the directions link takes you to Google Maps, where Google’s own privacy practices apply. The same is true when you choose the Facebook link.
Security
The site uses HTTPS, restrictive browser security headers, server-side validation and limited form fields. No internet transmission or email system can be guaranteed completely secure, so sensitive information should not be sent through the callback form.
Children’s information
The website is intended for adults arranging dental care for themselves or their families. Children should not submit the callback form without the involvement of a parent or legal guardian.
Changes to this notice
This notice may be updated when the website, form or service providers change. The review date at the top of the page will be updated when material changes are made.
